Has a chip on the back of the plate, but it's not visible from the front.  Has a small black spot next to one of the flowers. 

 I have no idea what type of plate this is (ie what it's function is).  If anybody has any idea, please let me know.  Thank you.

Replacements.com has your plate pattern listed, but I am not sure which plate size is yours. Follow the link below, you can find the right size plate. http://www.replacements.com/webquote/LIACHR.htm
